You should receive email notifications from Physical TheraPT confirming your appointments. Please click to add these sessions to your online calendar, as they will not auto-populate. 

We'd like to complete your new patient paperwork and intake questionnaire prior to your evaluation.  You can access our new patient paperwork and questionnaire using the buttons below. Please review this information, and submit at your earliest convenience.

What to Expect for your Evaluation

Plan on your first appointment lasting approximately 60 minutes. It will include a review of medical history, therapeutic exercise, as appropriate based on clinical presentation, and discussion of plan of care. 

You will have access to the facility's locker rooms, should you need to shower after your session. All amenities are provided. 

What to Bring to your Evaluation

Please wear athletic clothing and footwear comfortable for movement and exercise. Bring any braces or shoe orthotics you wear regularly. 

If applicable, arrange to bring copies of imaging reports, surgical reports, physician prescriptions and/or referrals, and any other relevant medical paperwork.

Payment 


 

Treatment rates are $200/evaluation and $165/follow-up sessions for physical therapy, and $155/evaluation and $135/training session for athletic training.  Payment is expected at the time of service, in the form of cash, check, credit card or HSA/FSA cards. 

Cancellations with less than 24hrs notice will be subject to 100% fee. 

We are an out-of-network provider with all insurance companies. If you have a PPO and would like to pursue reimbursement based on your policy, please let us know so we are able to provide itemized receipts. For ease of reimbursement, we recommend using Better. 

 
 
 
 

Better makes it simple to get paid back by your health insurance. Simply take a photo of your bill with the app, and let Better work with your insurance. Better charges 10% of the money you get back. If a claim is applied to your deductible or cannot be covered by insurance, the service is free.
